The former Burnley Bobcat could follow in the footsteps of previous winners Wayne Rooney, Tom Daley, Andy Murray, Amber Hill and Claudia Fragapane if he is presented with the accolade.
The 18-year-old, who won gold in the 200m freestyle at the IPC European Championships in March, won silvers in the 200m S14 freestyle and S14 individual medley at the Rio Paralympic Games this year, setting British records in both events.
The top three will be named on Blue Peter on Thursday.
Tom has been shortlisted alongside Team GB members Amy Tinkler, Ellie Downie and Rebekah Tiler as well as Paralympians Ellie Robinson, Jess Stretton and Lauren Rowles.
Cricketer Sophie Ecclestone, rugby player Keelan Giles and footballer Georgia Stanway complete the list.
