Leanne Townley, floating support co-ordinator at Calico, said: "I am delighted that the BeWell team have been shortlisted for Care and Support Team of the year. They have worked incredibly hard to support individuals to develop a personalised care plan that will enable them to have more choice and control over their care and to live a happy and fulfilling life.’

The BeWell service is a lifeline to those involved, changing the way they access care and support. By doing so, people with long-term health conditions and disabilities can shape their lives by making the key decisions about their own health, wellbeing and care.

As the NHS faces unprecedented demand, schemes like BeWell are attempting to provide relief. With those suffering long-term health problems currently accounting for 70% of NHS spend, a wellbeing-centred approach aims to deliver improved health and wellbeing, quality of care and better use of resources.
